Focus on quality of life

Though we all have aging in common, the aging process is different for everyone. Biological, social, psychological, environmental, and even economic factors affect how we age. As we grow older, both our physical and mental health will affect our ability to live independently. Tennova Healthcare created the Geriatric Assessment Program (GAP) to provide early intervention for age-related conditions, and to help older adults live the highest quality of life possible.

GAP for senior health

GAP is a multidisciplinary, outpatient assessment and treatment program to address the emotional, social, and functional health of adults who are 55 and older. By detecting early changes and providing appropriate and timely intervention, we can help seniors stay healthy. The program includes:

  • Evaluation – A team of doctors, nurses, and clinical social workers conduct a comprehensive assessment to screen for medical conditions, memory loss, depression, and social and functional issues.
  • Recommendations – The care team provides recommendations based upon an individual’s assessment results.
  • Treatment – A comprehensive treatment and maintenance plan will be designed according to individual needs. We can provide care for patients experiencing the onset or progression of dementia, anxiety, Alzheimer’s disease, and much more.
  • Additional services – We can provide medication assistance, letters of competency, or preadmission evaluations for nursing home placement as needed.

Who can benefit from GAP

GAP patients must be age 55 or older and meet at least one of the following criteria:

  • Memory problems that affect ability to function or live independently.
  • A sudden or gradual decline in the ability to perform daily activities independently or care for oneself.
  • Persistent feelings of depression or anxiety, with or without complicating medical or physical illness.
  • Consideration of a more supportive living situation, such as an assisted living program, nursing home, or other housing option.
